Community Care Coordination Learning Session

Join a broad group of community stakeholders for an informational session that brings national experts in to examine developing an Iowa Community Care Coordination solution. The event will be held at the Embassy Suites in Des Moines, Ia., from 10 a.m. to 3 p.m.

This session will feature a number of individuals with expertise in community care coordination and social determinants of health. The day will conclude with a discussion on the value and potential of Community Care Coordination solutions for policy makers based on other state's experiences.

Key legislators will be invited to participate in this discussion.

This event is being brought to Iowa through a partnership between Iowa Medicaid Enterprise, Iowa Collaborative Safety Net Provider Network, Iowa Primary Care Association, Iowa Association of Rural Health Clinics, and the University of Iowa Public Policy Center as a part of a National Academy State Health Policy (NASHP) Medicaid-Safety Net Learning Collaborative.
